Town of Avon, WI — Authorities have officially identified the woman who lost her life in a devastating single-vehicle crash last week as Madalyn M. Samonig, a 26-year-old mother of four from Orfordville. The Rock County Medical Examiner’s Office confirmed her death following the collision, which occurred on Thursday, April 10, 2025, near the intersection of West Skinner Road and South Moraity Road.

Emergency crews were called to the scene in the late afternoon hours after Samonig’s vehicle, which was traveling eastbound, reportedly veered off the roadway for reasons still under investigation. The car struck a tree with significant force. Samonig was pronounced dead at the scene.

In the vehicle with her were four passengers—one adult male and three children—who sustained injuries and were transported to a local hospital for treatment. While the extent of their injuries has not been publicly disclosed, officials confirmed that the passengers were alert when first responders arrived. Their names have not been released, in part to protect the identities of the minors involved.

No foul play is suspected, and the Rock County Sheriff’s Office continues to investigate the cause of the crash.
