Nevada Choate Obituary: Beloved Mentor, Ohio dad of two named as victim in horrific Madison Township car accident on State Route 528

MADISON TOWNSHIP, OH — The community of Madison Township is grieving the loss of Nevada Choate, a devoted father of two and respected mentor, who was tragically killed in a violent car crash along State Route 528 on the evening of Sunday, June 15.

According to the Madison Township Police Department, emergency responders were dispatched around 9:00 p.m. to the 5000 block of South Madison Road, where they discovered the wreckage of a single-vehicle crash that had ejected both the driver and passenger. Authorities say the vehicle veered off the west side of the roadway, struck a driveway culvert, and went airborne before violently rotating and coming to rest. Both occupants were thrown from the vehicle during the crash.

Emergency crews from the Madison Fire District arrived quickly and transported both men to University Hospitals Geneva Medical Center. One of the victims, identified as Nevada Choate, was later pronounced dead at the hospital. The other was flown via medical helicopter for advanced treatment. His condition has not been publicly disclosed.

Choate, who lived in Mentor, Ohio, was known by many as a compassionate individual, a strong presence in the lives of his two children, and someone who consistently gave back to others. He was often described as a mentor, role model, and pillar of strength for his family and community.

In the wake of the accident, the Lake Regional Traffic Crash Reconstruction Unit was called to the scene to assist in the investigation due to the severity of the crash. Madison Township Police, who are members of the crash unit, remain actively involved in determining what led to the deadly incident. Investigators have stated that speed is believed to be a contributing factor, though the official cause has not yet been confirmed.

South Madison Road was shut down between River Road and Griswold Road for roughly four hours Sunday night as investigators worked to reconstruct and document the scene.

As news of Choate’s death spreads, tributes have been pouring in on social media from friends, coworkers, and family members. Many remember him for his generosity, reliability, and quiet leadership — someone who inspired others simply by how he lived.

Nevada Choate is survived by his two children, whose lives he centered around, as well as numerous loved ones who are now navigating this sudden and devastating loss.
