Lysander, NY – A tragic motor vehicle crash on Oswego Road in the town of Lysander has left one driver dead and two others injured, including a young child.
According to the New York State Police, the accident occurred on March 31, 2025, at approximately 8:19 a.m. when a Chevrolet Express cargo van crossed the centerline and collided head-on with a Jeep SUV traveling in the opposite direction.
Victims Identified
The driver of the cargo van, Michael J. Simons-Iocona, 28, of Williamstown, NY, sustained critical injuries in the crash. Emergency responders transported him to Upstate Medical Center, where he succumbed to his injuries.
The driver of the Jeep, Tammy L. Piscitelli, 54, of Lysander, NY, suffered serious but non-life-threatening injuries and was also transported to Upstate Medical Center for treatment.
Additionally, a 3-year-old passenger in the Jeep was taken to the hospital as a precautionary measure for evaluation.
